Situations which are "locked" in. The best motivation in the world will not help systems that are designed to prevent change
The education system in most countries is a prime example of a "locked in" system.
The subjects taught are largely out of date and are only there because they were there yesterday and the day before. There are much more important and relevant subjects which are not taught at all.
The system is often tied in to examinations set by universities for their own purposes. Teachers have to do what they are supposed to do
